The Psychological Safety Questionnaire – Hive-Mind: Keeping your workplace healthy

This questionnaire is based on Dr Amy Edmondson’s ‘The Fearless Organisation’ Psychological Safety Matrix.

Rate your agreement with each of the statements below using the 1-5 Likert scale indicated (1 being ‘strongly disagree’, 3 being neutral, and 5 being ‘strongly agree’).
